Lost 96 Ghost of Madden Randy Moss after replacing w/96 OVR power up pass

Summarize your bug I reached level 85 and received a 96 OVR power-up pass. I used the pass to replace the 96 OVR Ghost of Madden Randy Moss item, which I already owned and had used to power up Moss to a 97 OVR (not including chemistries, which made him a 98). After successfully replacing the actual 96 OVR Moss with the power-up pass (at which point I had the identical power up Moss as before), the Ghost of Madden 96 OVR Moss didn't appear in my binder as it should have. This is an expensive card that I have now lost because of an apparent bug. I would like the card replaced.

Just a note that although I selected "Solo Challenges" as the part of the mode where the bug occurred, the bug was actually having a player disappear from my binder. There are no options relevant to this, so I just selected an option to be allowed to submit this bug.
