Steps: How can we find the bug ourselves? Have any two played households in the game. While playing household A have someone in household B die.
What happens when the bug occurs? The played sim in the inactive household does not leave behind an urn or tombstone
What do you expect to see? I know for certain that in April of 2022 if a played sim in an inactive household died, the urn could be found at the front door. I used this to create generational cemeteries in a game at the time.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.

Please describe the patch or change you made. Installed patches and base game updates form 4/17/22 to 06/22/22 as well as the Werewolves Game Pack
I know for certain that in April of 2022 if a played sim in an inactive household died, the urn could be found at the front door. I used this to create generational cemeteries in a game at the time. I stopped playing for a couple of months and came back with the Werewolves Game Pack. Now the played sim in the inactive household does not leave behind an urn or tombstone. This is very distressing as I like to "retire" my sims to a retirement home when they become elders, then I go to the household when a played active sim is notified of death and move the urn to a "Simetery" i almost always have build in Newcrest. I cannot do this anymore. To have all the ancestors stay in the active household till they die of old age will fill up the family very quickly.
