Summarize your bug Playing NS into P3; When I clear P3 Talzin has isolate on her. P4 begins and all previous buffs/debuffs are removed. There are no icons etc. When talzin is defeated she cannot be revived just as if she still had isolate on her when she died. I have had this happen several times.
Steps: How can we find the bug ourselves? Play P3, get talzin isolated. Beat the phase and go into P4 with isolate on talzin. Get talzin killed before traya uses isolate. try to revive her.
