What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. Jumping off map during wraith ult and dying in void shift. Then got respawned.

How many matches had you played in a row before the issue occurred? Thousands. It's a recreatable exploit bug.

What happens when the bug occurs? Wraith is stuck in her portalstate, and can cancel cooldown on her tactical ability by using the melee button.
What should be happening instead? Wraith should be respawned in her normal state.
Steps: How can we find the bug ourselves? Step 1: Land somewhere on the map where you can die by jumping off. Step 2: Have your ultimate ready. Step 3: Start placing your portal, then jump off the map, and use wraiths' tactical ability before dying from the fall. (You have to be in the void when dying). Step 4: Have a teammate repick your banner and respawn you at a nearby beacon. Step 5: You are in Wraith's portalstate and can use wraith's tactical without cooldown by using the melee button ("v" by default) while already in the void.
