Steps: How can we find the bug ourselves? Be an Alien Sim (not disguised) and try to "buy and wear" (or buy, then wear) a tin foil hat or novelty shirt at the Curio Stall
What happens when the bug occurs? The Sim becomes (permanently, until changed in CAS) shirtless or hatless in that outfit
What do you expect to see? As with the disguise + Human Sims, them to wear the item purchased
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
Tried multiple saves to be sure it wasn't someone with a stuck buff or something.
You're able to go into CAS and choose the items manually after purchasing, just not with the stall interaction.
And yes, this does mean that alien ladies end up with uncensored, naked (albeit nipple-less) breasts.
