This just happened to me when one of my teens changed into his scout uniform. After his meeting, had him change out of the uniform and the other outfits are fine.
I deal with a similar glitch in-game, it's appeared multiple times for random NPCs and seems to be the same looking graphical glitch
Please use this thread if you have this issue on a game with no mods/cc
To fix, set the sim details on very high

Which language are you playing the game in? English
How often does the bug occur? Rarely (0% - 9%)
What is your current game version number? 1.52.100.1020
What expansions, game packs, and stuff packs do you have installed? All of them
Steps: How can we find the bug ourselves? Not sure. NPC spawned unnatural white spots. This was shortly after using freerealestate, moving an 8 sim household in and trying to make them get jobs simultaneously. They all looked at their phones but did not continue to get a job. After having 4 or 5 sims get a job in the medical field, an NPC spawned that was an adult sim in a scout outfit and a checkered print over its entire body.
What happens when the bug occurs? checkered print over NPC entire body from hat to shoes.
What do you expect to see? no patchy overlay
Have you installed any customization with the game, e.g. Custom Content or Mods? Never used.
Did this issue appear after a specific patch or change you made to your system? No
Guess my Llama Scouts are still not meeting. Was just wondering if this was still an issue. Yep.
