Hmm alright since we can only get the crash to happen through apex lets use it as a stress test. (games can stress a GPU in different ways that vary between games)
Download MSI after burner here: http://download.msi.com/uti_exe/vga/MSIAfterburnerSetup.zip
and setup the settings like so:
so max out the temp and power and untick the magnet icon to the left if enabled.
set the core clock to -100
Click the tick button and leave Msi running in the background.
Now play Apex and see if it crashes.
Also do you have any old GPU's lying around?

Used the Msi program, if anything it made it crash faster? Not sure if the speed meant anything as it has happened at the menu before multiple times.

Okay try each test and post the results in the numerical order they are posted in:
1.Set the core clock to 0 and check again how does it compare to the -100 test.
2.put everythong back to 0 but set the core clock to -100 and test.
3.Max out the core voltage, if its faded out you can click the gear cog icon and copy my settings like so:

Also may i get the exact model of your PSU?

Test-1: Crash received within 10-15 mins sitting in training mode
Test-2: Crash within less than 2 minutes upon booting the game. received at menu
Test-3: Crash 5-10 mins sitting in training mode.
All the same error.

Ahh i forgot to mention.
Make sure the power and temp limited are maxed on test 3 with the maxed voltage and 0 on the gpu.
And do you have another PSU by chance? even if it is one you can borrow for a bit out of another system. (500w or higher)
I have reason to believe your PSU may not have enough Amps in the 12v rail in order to run the GPU
and im trying to figure out if its the PSU or the GPU.

Yo, haven't had the chance to play apex recently but saw the recent patch and gave it a try.
Just had one whole session with 0 crashes whatsoever. Game feels so smooth aswell, although I do get small 'hitches' from time to time, which im fairly certain can be fixed through a DDU uninstall reinstall. Thought I would pop back in just to say the * has been resolved thankfully.

Be sure to unistall MSI afterburner and rivatuner to tidy up.
(Just as a precaution nothing major)
Patch 1.1 Resolved this issue at time of writing.

Just hopping on here to say i just received it again.
Dont want people to think I actually solved the problem. Gonna be playing other games in the meantime till next update, heres to hoping a fix comes then.
