What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. Using the death totem

What happens when the bug occurs? Wraiths character model doesnt load
What should be happening instead? Wraith should have a normal model
Steps: How can we find the bug ourselves? (I tried it in 1st and 3rd person and it works in both) You play as Revenant and place down your death totem and use it. Then you switch to Wraith and you are still in the Revenant model.
