this outfit looks fine in my game.
Could you upload your Sim to the galley to see if it's an issue with certain face settings ?

also, when the sim aged up to a teenager, all of her skills reset, can you help with this too please

The skills for teens and children are different. So unless you maxed the children skills, that's what will happen.
I can duplicate the issue on your Sim. Looks like an issue with the cheeks and the glasses.
