How is it possible that 2% chance in packs = never happens yet 2% chance to dodge in combat happens constantly, and often back to back? 

Additionally, what is the actual drop rate for g12+ gear on fleet nodes? 15 battles, 0 gear is the norm, 20 battles (200 energy) for 1 piece is also normal.

This appears to be a case of bad RNG as a 2% chance could be expected, but is not guaranteed 1 out of every 50 times. In battle, there are also Buffs that can increase dodge chance such as Evasion Up and Foresight as well as individual character stats.

I'm willing to bet that your sample size for attacks is much, much larger than for buying packs.
