My sim was wide awake when she woohooed with her elder husband. The game forces a moment of sleep after woohoo (which is the culprit I suspect). The death action overroad my sim waking up. She slept through her husband's death, Grim's appearance and her child coming in and crying his heart out. I could X out the sleep icon, but she would never wake until everything was over and there ewas only an urn. She did not seem to realize her husband had died at that point either.
I reloaded and used the teleport cheat while the couple woohooed. I was able to get the sleep icon to X out and my sim to woohoo, but the death and Grim's appearance happened off screen. When she teleported, her husband was gone and only an urn remained.
This could probably be solved if sims did not default to sleep after woohoo unless they were 50% or less on energy.
I run a vanilla game with only the testingcheats always on mod.
