Sorry, but nothing transfers inbetween different platforms. The only transfers allowed are stats and Premium (not seperatly purchased DLC, just Premium itself) inbetween Xbox 360 > Xbone and PS3 > PS4 and nothing will go from Xbox > PS (or vise versa) or any console > PC (or vise versa).
You will have to re-purchase the game and Premium / DLC on the PS4.
