Hello! I cant find my puppy either and the link for the form is not working anymore, where can I report IT?

Go into your game, open the menu, and click on the green?. That should bring up the form. Please note that all the boxes need to be filled out. If you don't have a Facebook or google account just copy and past the login URL.

Me too!  The puppy disappeared when I tried to take it home and now not only can I not finish this quest, I can't do any others until I get rid of this one.  I sent a request and have heard nothing so far.
