Progress failed

When I play deathmatch my wepons and unlocks are still stuck on level one. On Conquest all my gear is still up to date. And when I play campaigne the game eventually gets stuck on a loading screan and I am forced to restart my system, all my save data is lost and I have to restart the campaigne. Do I just have a bad disc or is there something I can do to fix this problem?
