Phantom Prospect assignment not tracking

So I am on PS4 and have input the 3 passwords into the "console" and now says: 

"Phase 4 now active. Physical access restriction in place."

On the battlelog I have it set to track and in game under the assignments it shows the the requirements checked and what is needed to complete it. When I started BF4 there was a popup that indicated that Phase 1-3 have been unlocked. Despite this I am not able to make any progress on it.
