You can buy Battlefield 4 Premium for PS3 at your local retail store and redeem the code or alternatively you can go to PSN Store on your PS3, search for "Battlefield 4 Premium" and follow the steps on the screen to purchase it.

Local retail stores don't have it. I searched online and the Sony site requires a U.S. address to purchase. Is this different than logging in through the PS3?

Please log-in to your account through your PS3 console and open the PSN Store. You can buy Battlefield 4 Premium from any country you want, so this shouldn't be an issue unless you have provided false personal information on your PSN account.
