battlefield3 premium without b2k

i bought bf3 premium today and i dont have access back to karkand expansion pack

What i must do to get b2k ?

You didn't say which platform you are on so I'll post a generic description. After you purchase Premium or redeem the Premium code if ou purchased the physical copy of the BF3 Premium edition make sure that you restart BF3 and when you reach the main title screen you will receive a pop up stating you are now premium and you will have a Premium background.  Once at the main title screen download the DLC expansions covered by Premium via the BF3 in game store on the Premium tab (this is where you will find Back to Karkand), then close and restart BF3.  After the expansions are downloaded and installed restart BF3, at this point you will receive a series of pop ups stating that you now have each expansion.  Now from the main menu go to multiplayer, open server browser and then open the server browser filter and check all new maps and game modes or you won't find any games on the new expansions.

You are correct, Back to Kaharkand is contained in a file called Battlefield 3 Update which is 2.1 GB in size.  If you are Premium start BF3, go to the BF3in game store then go to the Premium tab of the BF3 in game store and you can download B2K for free there.

Here is a listing of files that you need and don't need on Xbox:

Mandatory Files:

Battlefield 3 Title Update (Auto-Download) - 201 MB

Muiltiplayer Update 6 - 2.0 GB

Minimum HDD space require to play Multiplayer:  2.2 GB

DLC Specific files:

End Game - 1.8 GB

Aftermath - 1.6 GB

Armored Kill - 1.6 GB

Multiplayer Update 3 (Close Quarters) - 2.4 GB

Battlefield 3 Update (Back to Karkand) - 2.1 GB

HDD space required to play ALL the DLC:  9.5 GB

Optional files for better performance:

Battlefield 3 Disc 1 - 7.3 GB

Battlefield 2 HD Content - 1.5 GB

Old files you can delete:
Multiplayer Update 5 - 1.8 GB

Multiplayer Update 4 - 1.44 GB

Rent-A-Server\E-sports-update - 1.02 GB

Total recoverable space for deleting obsolete files:  4.26 GB
